There several types of documentation about the resident that the recreation therapist is responsible for completing. Each component must be carefully written and reflect the actual condition and needs of the specific individual resident in your facility. This documentation must also meet all requirements defined in the state and federal regulations, the five step process of care planning, legalities affecting documentation, terminology and abbreviations used in documentation.
